Once you have scheduled your first appointment, please select the appropriate forms to review and fill out. Adults bringing their minor children for services will select "Child and Adolescent Intake Forms." Adults over the age of 18 will select "Adult Intake Forms." Everyone please review the Notice of Privacy Practices. You may also arrive 10-15 minutes before your first appointment to fill out paperwork.
